This programme offers support and advice to you and your family if you have had a Stroke, suffer from Hypertension, High Cholesterol or Heart Disease OR should you have been hospitalised with a cardio vascular condition or have recently undergone cardiac surgery. Support and advice are a phone call away.
You are always encouraged to follow the advice given to you by your doctor. However, there are times when you may be unsure about the way you feel or symptoms you may experience following a cardiac admission. This is the time to make use of trained, professional support and / or advice.

  • Education on diet & lifestyle changes to enable you to maximise your health, going forward
  • Advice and assistance in registering your condition for Chronic medication, consultations etc.

The programme co-ordinator automatically contacts you if you have been hospitalised with a heart related condition or have undergone cardiac surgery. However, members as well as their family members, are welcome to call during office hours (08h30 – 16h30) for support and advice on 0860 100 544.
